Aha!
I think I found the trouble.. I was using an $OPERATION$ = "CREATE"
qualification along with Execute On of "Copy to New" to try and capture
either situation.
Each works fine by itself, but "Copy to New" isn't considered a "CREATE"
operation, so the qualification fails? (Seems odd to me since a new record
is indeed being created).
Going to try breaking this workflow in two seperate links and see if that
helps.

You could do a couple of things
1) On submit check to see if these fields are Null. If
you suspect that your users are doing this on purpose
pop a message up that says
"If you used Ctrl-E, You must close this form and open a new
form, then re-enter your information"
A little harsh, but after having to do this several
times those users should get the message not to use ctrl-e :)
2) On submit create a al or filter that will reset those fields with
whatever values that were original set.

I have some workflow in place to set certain hidden fields on form
startup. (For historical reasons, I don't want to make these required
fields). I've recently discovered some users who have been able to
submit tix with null values for these fields, and I can only attribute
this behavior to the "Clear all" functionality - (I haven't confronted
the users yet, though this is my next step).
Is there a way to prevent "Clear All" ? Or at least prevent it from
clearing certain fields?
