We had performed an Oracle data import following a machine rebuild. During that process the control table became corrupted, with the next available schema Id field set to 3!

When the field was set to the last id+1, we had no problem.

We had a similar problem while in the process of setting up our development server. Since in this process, we performed a DB migration from our development box, many of the indexes were off sync, and we had to manually correct them at the DB level. The was NOT a fun task, and we performed the migration 3 times before realizing what was causing it.

To resolve the problem (if you are performing a db migration/export), make sure that ARS is completely shut down first, on your source server before taking the export.

ARERR382

The error guide describes this as:

The value(s) for this entry violate a unique index that has been

defined for this form

The administrator has defined one or more unique indexes on this form. The

values on the entry you are submitting or modifying duplicate the field (or

fields) defined in the unique index. Review the values for the entry to ensure

that values that should be unique are, indeed, unique.

Any ideas why this would occur on the creation of a form?
