Try this. Open form in Remedy Administrator. From menu choose
Form->View->Set_Tab_Order. Change someting in fields order and save form. If
it help, don't ask why...
Sometimes Remedy Administrator show message when you open form. Something
like appearance is not correct and it will be improved after you save form.
After this forms also return to its original form.

I have reinstalled the fonts file for Windows NT and deleted all .arf and
.arv files. In addition I have reinstalled the user tool on both my machine
and one of the effected machines with no success. I installed the Admin
tool on one of the machines with the distored view and the form appears
correctly when viewing it through the admin tool. I also removed the form
from the server and imported back but that did not work either.
