Can any one let me know what are the Good no of indexes per form and to how many fields? is there any limit?

No thers are no limits but Indexes should be built only for fields which are frequently searched.If Indexing is done on lot of fields it will lead to perfomance issue.DB will be very slow so the tool.

I need to correct the previous point the indexing will always increase the performance of DB, the problem where it arise is when any unfortunate database corruption happens and if you try to restore it. It would take more time, so it better to make indexing so that it will boost the system performance and all unqualified will not hamper the performance of the system. Hope nothing should go wrong like that.
